Documents that shall be presented for the statement (appeal) of traffic violation cases are as follows:
(a) The original copy of the traffic violation notice.
(b) Statements (appeal) (stating the plate number of violation vehicle, notice number, reason, name, contact number and address).
(c) If you have evidential photos, please attach the original copies of them or other supporting evidence.
(d) If the statement maker (appealer) is not the disciplined subject, a letter of appointment from the disciplined subject shall be attached.
