Notes on applying for dashcam footage rewards
1. Filling out application form
Dashcam footages provided by the public are helpful for traffic accident cause determination. Upon receiving the reward issuance notice issued by Taipei City Traffic Adjudication Office, please fill out the application form including full legal name, contact phone number, address, personal ID number, and license plate number. Sign the declaration. Should the applicant be a foreign person, please indicate nationality and passport number.
2. Preparing photocopies of IDs and bank passbook cover
3. Application deadline
Within “three months” after the next day the issuance notice is served, mail the above-mentioned documents or submit these documents in person to the Accident Cause Determination Section of Taipei City Traffic Adjudication Office for issuance procedures (Address: 7th Fl., No.92, Sec. 4, Roosevelt Road, Zhongzheng District, Taipei City 10091; Tel: (02)2365-8270 Ext. 818). Please note that delivery to the Office after the deadline is regarded as having renounced the right to claim the reward.
